Are Anti-Wrinkle Injections Safe for All Skin Types? What Actually Decides It

Skin type is almost never the thing that decides whether anti-wrinkle injections are right for you. The injection goes into muscle, well beneath the layer where oiliness, dryness, sensitivity and pigment live. What actually decides it is your medical history, your medication and the anatomy of your face.

The question still comes up in almost every consultation, and it deserves a real answer rather than a reassuring one. Halcyon Medispa is an aesthetic clinic at 2 Hinde Street in Marylebone, London, and this piece sets out where skin type genuinely matters, where it does not, who should not have the treatment at all, and what the first two weeks actually look like. For the treatment itself, see our anti-wrinkle injections page.

The short answer: botulinum toxin is placed in the muscle beneath the skin, not in the skin, so it does not act on melanocytes and carries none of the pigmentation risk that surface lasers and peels carry on richer skin tones. Oily, dry, sensitive and combination skin all respond identically. The real screening questions are pregnancy and breastfeeding, neuromuscular conditions, certain medications, allergy to the product and active infection at the injection site. Treatment takes about 30 minutes with numbing cream, results appear at three to five days and settle at two weeks, and they hold for around three to four months.

Where the injection actually goes

Expression lines are creases pressed into the skin by muscles pulling underneath it, over and over, for decades. Frown lines come from the muscles between the brows, forehead lines from the muscle that lifts them, crow's feet from the ring of muscle around the eye.

Botulinum toxin does not treat the crease. It temporarily reduces the signal from nerve to muscle at the points where it is placed, so the muscle pulls less hard, and the skin above it stops being folded several thousand times a day. The line softens because it is no longer being made.

This is why the skin type question, asked in good faith, is aimed at the wrong layer. The product is delivered past the epidermis and the dermis into muscle. Whether your surface is dry, oily, thin, thick, pale or deeply pigmented does not change what happens in the muscle.

Skin tone, pigmentation and why toxin is different

The worry behind the question is usually a specific and well founded one: on richer skin tones, some aesthetic treatments carry a real risk of post inflammatory hyperpigmentation, a dark mark left behind after the skin has been injured and healed. Anyone who has watched a peel or a laser leave a patch darker than the skin around it is right to ask before they consent to anything else.

That risk comes from injury to the epidermis, where melanocytes sit. Heat, acid and abrasion all trigger those cells into overproduction while the skin repairs. A fine needle passing through to the muscle causes no such injury across the surface. There is no thermal energy, no chemical exfoliation and no controlled wounding of the pigment layer.

So the pigmentation conversation is a real one and it belongs to the surface treatments. It does not belong to toxin. What darker skin tones do need is a practitioner who is unhurried about surface treatments, which is a different appointment and a different discussion.

Oily, dry, sensitive and acne prone skin

None of these change how the toxin behaves. They do change small practical details.

  • Oily skin. No effect on the treatment. Skin is cleansed with antiseptic before injecting regardless.
  • Dry skin. No effect. Dryness is a barrier problem and worth treating in its own right, but it is unrelated to what happens in the muscle.
  • Sensitive skin and rosacea. Expect a little more visible redness for the first hour, and small raised bumps at each point that settle quickly. The result is the same.
  • Acne prone skin. Active inflamed spots at the injection site are a reason to move the appointment, not a reason to avoid the treatment. Nobody injects through infected skin.
  • Thin, crepey skin. Worth flagging, because relaxing a muscle under skin that has lost its own quality can reveal texture the movement was hiding. That is an argument for pairing it with skin quality work such as Profhilo, not against treating.

The four treatments ranked by pigmentation risk

If you are weighing up what is safe on your skin tone, this is the comparison worth having in front of you.

TreatmentWhat it acts onPigmentation risk on richer skin tonesWhat that means in practice
Anti-wrinkle injectionsMuscle beneath the skinNegligible, no epidermal injurySkin tone is not a limiting factor
Aerolase laserTargets within the skin using lightLower than older lasers but present, depends on settings and operatorAssessment and test patching matter
Chemical peelsControlled exfoliation of the surfaceReal, and depth dependentStart conservative, prime the skin, follow aftercare closely
HydraFacialSurface cleansing and hydration, no woundingVery lowSuits most tones and skin conditions

The pigmentation question is a good one asked at the wrong appointment. Ask it before the laser, not before the toxin.

Who genuinely should not have it

These are the exclusions that matter, and not one of them is about skin type.

  • Pregnancy and breastfeeding.
  • A neuromuscular condition affecting the signal between nerve and muscle, which should always be declared.
  • A known allergy to the product or any of its components.
  • An active infection or inflamed skin at the site to be injected.
  • Some medications, particularly certain antibiotics, which can interact. Bring your list rather than trying to remember it.

There is also a group who can have it and often should not bother: anyone hoping it will lift a heavy brow, and anyone whose lines are now etched into the skin at rest rather than appearing on movement. Toxin softens those static lines less than people expect, because the crease has become a feature of the skin itself.

What actually drives your risk, and it is not your skin

Nearly every poor outcome from anti-wrinkle injections comes from one of three things, and this is worth saying plainly.

  1. The dose and where it went. A drooping eyelid or a heavy brow is a placement and dosing problem, not an allergy and not bad luck with your skin.
  2. Who held the needle. The face is a crowded piece of anatomy and the margin between a good result and a strange one is a few millimetres. At Halcyon Medispa treatment is medically led, and the assessment before the first injection is the part doing the work.
  3. What was not declared. The medication you forgot, the condition you thought was irrelevant, the treatment you had elsewhere last week. Practitioners cannot screen for what they are not told.

This is a stronger opinion than most clinic pages will offer: choosing a good injector matters more to your safety than anything about your skin, and price is a poor proxy for either. If a quote is markedly below GBP 200 for a full area, ask what product is being used and who is administering it.

The session, start to finish

  1. Consultation and mapping. Your history and medication are taken, goals agreed, and you are asked to frown, lift your brows and smile so the practitioner can see which muscles are actually creating your lines. Photographs are taken for comparison.
  2. Preparation. The skin is cleansed with antiseptic and a topical numbing cream is applied.
  3. The injections. A very fine needle, several small injections per area, a brief sting each time. The injecting itself takes only a few minutes.
  4. Afterwards. A cool compress if you want one, aftercare instructions, and you leave. The whole appointment runs about 30 minutes and it is a single session.

Areas treated here include the forehead, the frown lines between the brows, crow's feet, a lip flip, the chin, the neck and jawline slimming. Related conditions such as masseter hypertrophy and excessive sweating are treated with the same product for different reasons.

The first two weeks

WhenWhat is happeningWhat you will see
Day of treatmentProduct beginning to bind at the injection pointsSmall raised bumps and mild redness for up to an hour. No change in movement yet
Days 3 to 5Effect startingThe first softening. Movement in the treated muscle begins to reduce
Around 2 weeksFull effectThis is the result. Judge it now, and this is when a review appointment is useful
Months 3 to 4Effect wearing off graduallyMovement returns. Most people book their next treatment around this point

If something looks uneven at day five, it is usually too early to call. If it still looks uneven at two weeks, ring the clinic on 020 7486 3225. That is exactly what the review appointment exists for and small adjustments are straightforward.

Aftercare, the rules that matter

  • Four hours: do not rub or massage the treated area, and stay upright rather than lying down.
  • 24 hours: no intense exercise, no facials or other skin treatments, and nothing tight on your head such as a swimming cap or a firm hat band.
  • Around the appointment: avoid alcohol and blood thinning medication, unless it is prescribed, in which case tell us rather than stopping it.
  • Any time: daily sunscreen, which protects everything else you are doing for your skin.

If you are also having filler, the order and the gap between them matter. Our piece on what to avoid before and after lip injections covers that in more detail.

What anti-wrinkle injections will not do

  • They will not tighten loose skin. That is a different problem with a different answer, such as HIFU or, where there is genuine skin excess, surgery.
  • They will not restore lost volume. A flattened cheek needs replacing, not relaxing.
  • They will not fully erase a deep static line. They soften it and stop it deepening.
  • They will not improve pigmentation, redness or texture. Those belong to laser, peels and facials.
  • They are not permanent. Three to four months, then a decision.

What it costs

Anti-wrinkle injections start from GBP 200 at Halcyon Medispa, with the exact figure depending on how many areas are treated. The consultation is GBP 150 and comes off the cost of treatment in full. Current pricing is on the treatment prices page.

Frequently asked questions

Are anti-wrinkle injections safe for all skin types?

For the great majority of people, yes. Botulinum toxin is injected into muscle below the skin and does nothing to the pigment producing cells in the surface layers, so oily, dry, sensitive and richly pigmented skin all respond to it the same way. What varies between people is not their skin type but their medical history, their medication and their facial anatomy, and that is what a consultation is for.

Do anti-wrinkle injections cause pigmentation on darker skin?

There is no mechanism by which they would. Post inflammatory hyperpigmentation follows injury to the epidermis, which is why lasers, peels and aggressive extractions carry that risk on richer skin tones. A fine needle passing through to muscle does not injure the epidermis in that way. The pigmentation conversation belongs to surface treatments, not to toxin.

Who should not have anti-wrinkle injections?

Anyone pregnant or breastfeeding, anyone with an active skin infection at the injection site, anyone with a known allergy to the product, and anyone with a neuromuscular condition affecting nerve to muscle signalling. Certain medications matter too, particularly some antibiotics. None of that is a skin type question and all of it is asked at consultation.

How long do anti-wrinkle injections take to work?

Nothing on day one. Most people see the first softening at three to five days and the full effect at about two weeks. Judge the result at two weeks, not before, and book any top up review after that point rather than during the first week.

How long do the results last?

Around three to four months at Halcyon Medispa, varying with how strong the muscle is, how active you are and how quickly you metabolise. People who treat regularly often find the interval stretches, because a muscle that has been relaxed repeatedly does not come back as strong.

Does it hurt?

A topical numbing cream is applied first and most people describe a brief sting per injection point rather than pain. The session takes about 30 minutes including preparation and is a single visit.

Can I have anti-wrinkle injections if I have sensitive skin or rosacea?

Usually yes. Redness and small raised bumps at the injection points settle within an hour or so in most people, and sensitive skin does not change how the toxin behaves in the muscle. Tell your practitioner if your skin flushes easily so the aftercare advice can be adjusted.

What should I avoid straight afterwards?

For the first four hours do not rub or massage the treated area and stay upright rather than lying flat. For 24 hours avoid intense exercise, facials and tight headwear, and avoid alcohol and blood thinning medication around the appointment unless prescribed.

How much do anti-wrinkle injections cost in London?

They start from GBP 200 at Halcyon Medispa. The consultation is GBP 150 and is redeemable in full against treatment. Current figures are on our prices page, and the amount depends on how many areas are treated.

Will my face look frozen?

Only if too much is used, and that is a dosing decision rather than an inevitability. The point of the assessment, where you are asked to frown, raise your brows and smile, is to map which muscles are doing the work so the dose can be placed where it is needed and kept off the muscles you want moving.

Ask the question at the consultation

If you have been putting this off because of something about your skin, bring it to the consultation and say it out loud. Dr Osman Bashir Tahir and the team at Halcyon Medispa, 2 Hinde Street, Marylebone, London W1U 2AZ, will tell you whether anti-wrinkle injections are the right answer for the lines you actually mind, or whether something else is. Our patient journey page explains what a first visit involves, and this piece covers what separates a medical clinic from a salon.
